Activated 3.64 in Kozelsk, Kaluga Oblast, under the 28th Guards Missile Division.

Controlled site BSP-22 with 3 R-9A silos - alert duty from 1965

In 1969 the BSP-11 and BSP-12 sites were transferred to the 289th Missile Regiment:

• site BSP-11 with 2 R-9A pads

• site BSP-12 with 2 R-9A pads

• site BSP-22 with 3 R-9A silos

The regiment stood down 1977, and was disbanded.

US designation 1960-1977: Kozelsk ICBM Complex (Launch Site F, later Launch Site 6). Type IIIB silos.
