Activated 7.3.62 in Krosno Odrzanskie, Poland, from the 4th Infantry Division.

Organisation 1962:

23.7.63 the 12th Multiple Rocket Launcher Battalion was activated.

17.7.66 the 65th Medical Battalion was activated, and the 25th Reconnaissance Company was renamed 25th Reconnaissance Battalion.

1.4.67 the 4th Supply Battalion and the 4th Repair Battalion was activated.

4.5.67 the 36th Communications Battalion was renamed 4th Communications Battalion.

10.11.67 the 24th Missile Battalion joined the division.

30.11.67 the 26th Anti-Aircraft Artillery Battalion was renamed 128th Anti-Aircraft Artillery Regiment.

Organisation 1970:

In 1972 the 97th Anti-Tank Artillery Battalion was activated.

20.11.89 the 11th Mechanised Regiment was disbanded, and the 18th Medium Tank Regiment was renamed 18th Mechanised Regiment.
