Activated 6.60 in Teykovo, Ivanovo Oblast, under the 197th Missile Brigade, and from 5.61 the 54th Guards Missile Division.

1963 with 2 battalions at two sites:

• site BSP-1 with 2 R-16 pads - alert duty from 11.11.63
• site BSP-2 with 2 R-16 pads - alert duty from 15.12.63

In March 1964 all battalions were upgraded to regiments:

• site BSP-1 became 771st Missile Regiment with 2 R-16 pads
• site BSP-2 remained as 604th Missile Regiment with 2 R-16 pads

In 1968 the BSP-3 and BSP-4 sites were transferred to the 604th Missile Regiment, now with:

• site BSP-2 with 2 R-16 pads
• site BSP-3 with 2 R-16 pads
• site BSP-4 with 2 R-16 pads

The regiment stood down 1976, and was disbanded shortly thereafter (1976).

US designation 1960-1976: Teykovo ICBM complex (Launch site D, later Launch Site 4). Type IID pads.
